PLEASE NOTE: YOU MAY ONLY REGISTER FOR ONE INTERSCHOLASTIC SPORT PER SEASON.

ALL ATHLETES MUST BE CLEARED PRIOR TO PARTICIPATING IN PRACTICE/TRYOUTS.  VISIT AND REVIEW YOUR FAMILY ACCOUNT TO VERIFY YOUR STUDENT IS CLEARED FOR PARTICIPATION.

TROUBLESHOOTING TIPS:
Do not create a new account if you have previously registered your student.  You are a returning user and need to log in your account in order to register. You will not be able to register your student in a new account if you have already created one.

If you are having an issue where clicking an icon brings you back to this page, please scroll to the bottom of the registration home page and look for be a link to go  “Back to Account” which will take you back to your family account or “Logout” which will log you out of Registration. From there you can click the icon and log in to your family account. 

Physical Examinations (New For 2024-25)

Students registering for athletic participation must have a current physical examination on file in the Health Services Office. 

Preferred Form: The State of Illinois Certificate of Child Health Exam form can be downloaded here.

To ensure clearance for participation, physicals should be:

• Completed and submitted to the Health Services Office no later than 5 school days prior to the first day of practice/tryouts. 
• Physicals are valid for 395 days from the date of the last exam.
• Please allow 3 school days from receipt for review and approval. 

Physicals can be submitted in the following ways:

• Upload your student’s physical to PowerSchool. This is the preferred way to submit your physical. Instructions for Uploading a Physical
• Physicals may be emailed to healthservices@nths.net (for Sophomores, Juniors and Seniors) or northfieldhealth@nths.net (Incoming Freshman only)

If your student’s physical expires during a season, you will be required to upload an updated physical exam via PowerSchool.
